In my opinion I think the start of the game should stay as it is, but along the game server time, like in crucial time tables, for example every 2-3 months there should be an election and if players are ok with their king fine, they will vote to continue, or if someone has better ideas of how to run the kingdom, like a duke he could challenge the king and overthrow him, that would make the game more interesting and solve that problem if there would be an inactive king, someone could challenge him and if he has not been active it would be easily defeated.
